Join PhyNet Health System for a rewarding career opportunity! As an LVN, you will be a valued asset to our team and an essential, integral member. Your role will allow you to work closely with MDs and FNP Family Nurse Practitioners, experiencing daily medical practice in a hands-on, face-to-face environment. The PhyNet clinic setting provides you an opportunity to champion better health care by being a "care advocate" for the local community. Our practice patient demographics age ranges from Pediatrics to Senior Adults. As the lead Nurse and assistant to the primary clinicians, your responsibility includes, but is not limited to, capturing physiological data, assisting with all medical follow-through, including care coordination, patient education, chronic care management, and transitional care management. Additionally, ongoing documentation of patients' progress in the EMR, including ordering and monitoring labs, assisting with care plans, providing patient education, and recording quality measures, plays a vital role in filling gaps in care. To better care for patients, PhyNet operates in a CareTeam environment. Medical professionals, MD, DO, FNPs, and various care management and clinical care coordination members and your role as nursing staff, will work together to provide comprehensive care for optimum health and wellness of patients.
Requirements:
The candidate must have a minimum of 2 years of medical background experience and be comfortable in a fast-paced clinical environment.
