Aveanna Healthcare, a preeminent provider of in-home medical and therapeutic services, is currently seeking compassionate and dedicated Licensed Vocational Nurses (LVN) specializing in low acuity care. This role focuses on providing professional nursing care to patients with minimal needs in their own homes, ensuring comfort and quality of life. Candidates will collaborate with a multidisciplinary team to tailor care plans specific to patient requirements and will be supported by our cutting-edge resources and continuous professional development opportunities. Responsibilities Assess the health condition and needs of patients with low acuity requirements through examinations and patient histories. Implement and monitor patient care plans tailored to the specific needs of the patients. Administer medications and other treatments as prescribed. Document patient care services and update records in compliance with the nursing and legal standards. Educate patients and their families on health maintenance and disease prevention. Coordinate with healthcare teams to ensure the effectiveness of treatments. Required Skills Proficiency in administering medications and understanding of treatment protocols Ability to operate medical equipment relevant to home care settings Strong communication and interpersonal skills Detail-oriented with robust documentation and patient management abilities
