In this role, the LVN will provide direct patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Responsibilities include patient assessment, medication administration, treatment support, and accurate clinical documentation. This position requires strong clinical skills, attention to detail, and the ability to work effectively in a fast-paced, collaborative environment. Experience with starting IV's a must! Responsibilities Perform patient assessments, including vital signs, basic physical assessments, and health history documentation using Electronic Health Record (EHR) systems such as eClinicalWorks Administer medications and injections in accordance with physician orders, including IV infusions and medication management . Assist with medical procedures, including sterile processing, while maintaining aseptic technique Collaborate with physicians and interdisciplinary care teams to support and implement individualized patient care plans Maintain accurate, timely, and compliant medical documentation in accordance with HIPAA regulations and ICD‑9/ICD‑10 coding standards Qualifications Current and valid LVN license issued by the state licensing board Prior clinical experience in a clinic setting preferred Proficiency with EMR/EHR systems, including eClinicalWorks Knowledge of medical terminology, anatomy, physiology, and diagnostic assessment principles Demonstrated competency in medication administration, injections, venipuncture, catheter care, and IV insertion/infusion Strong communication skills and critical thinking abilities to support safe, effective patient care in a fast‑paced environment
