GENERAL DESCRIPTION

Home health LVN or otherwise known as licensed vocational nurses are part of health professionals who are responsible in providing health interventions within a home-based setting. Their primary function is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency. Communications and documents clinical findings as per agency policy and procedures. Functions with the scope of practice for LVN and individual competencies.

WORK EXPERIENCE REQUIREMENTS

 Minimum of 1 year experience in an providing nursing care in the home health care environment .  Knowledge of the current Texas Board of Nursing Rules and Regulations  Excellent organization, multi-tasking skills, and verbal /written communication skills.  Proficiency in Microsoft Office software

EDUCATION REQUIREMENTS

• Licensed Vocational Nurse (LVN) licensed in the state of Texas
• One (1) year experience providing comprehensive skilled nursing care in the home health environment preferred.
• Individuals must pass the National Council Licensure Exam for practical nurses (NCLEX-PN)

JOB RESPONSIBILITIES AND DUTIES

 Furnishes skilled nursing services as assigned and in accordance the plan of care..  Prepares clinical and progress notes and submits to the agency according to agency protocols.  Accurately and effectively documents skilled observations and assessments, treatments and medications administered, patent/caregiver teaching, patient response, and communications with physician and RN of findings follow-up as appropriate.  Reports changes in a patient status to RN and physician.  Updates the medication list with changes.  Documents physician orders and submits to RN for review and implementation of the change in plan of care.  Assists the physicians and RN in performing specialized procedures in accordance with the plan of care, LVN scope of practice,
