Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders, including IV infusions and catheterizations. Manage acute pain and chronic conditions through appropriate nursing interventions and medication administration. Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures as needed. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral management. Support patients with complex needs such as ventilator management, tube feeding, or wound care while adhering to infection control protocols. Collaborate with interdisciplinary teams on case management plans for clients with developmental disabilities or those requiring hospice or palliative care. Document all patient interactions thoroughly in EMR/EHR systems while maintaining strict compliance with HIPAA and clinical confidentiality policies. Educate patients and their families on health coaching, medication management, nutrition, and post-discharge care instructions. Ensure safety standards are met by practicing aseptic techniques, first aid procedures, airway management, and sterile processing. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, skilled nursing facilities, or outpatient clinics. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and long-term care regulations. Experience with EMR/EHR systems like Epic, Cerner, Athenahealth or similar platforms for documentation and patient records management. Demonstrated ability to perform IV insertions/insertion of PICC lines or other vascular access devices; experience with IV infusion therapy preferred. Familiarity with caring for geriatric patients and those with complex conditions such as infectious diseases or acute pain episodes. Excellent communication skills for caregiver interaction and patient education; ability to work effectively within multidisciplinary teams. Basic life support (BLS) certification required; additional certifications such as ACLS or PALS are advantageous.
