Job Summary:
The in-office Licensed Vocational Nurse (LVN) works alongside the RN Case Manager to keep physician orders, chart documentation, and coordination tasks current for the RN Case Manager's active caseload. This role handles order and signature follow-up, documentation review, supply and equipment coordination, and day-to-day communication with physician offices, patients, and field staff — freeing the RN Case Manager to focus on clinical assessment, plan-of-care decisions, and patient-facing care.
Responsibilities:
Track and follow up on physician orders and Detailed Written Orders (DWOs) until each is signed, dated, and on file Review chart documentation for completeness, and prepare recertification and Start of Care (SOC) paperwork ahead of deadlines Maintain the patient chart, keeping required forms, consents, and physician communications current Coordinate supplies and durable medical equipment, and support scheduling continuity by flagging upcoming SOC, recertification, and discharge dates Assist with referral intake follow-up, and liaise with field staff on physician order updates, supply arrivals, and schedule changes Serve as an office point of contact for physician offices, patients, and families, maintaining accurate, organized records of communications and follow-up
Required Experience:
Must have a current, unencumbered Licensed Vocational Nurse (LVN) license in the state of Texas, in good standing with the Texas Board of Nursing, and be a graduate of an accredited practical/vocational nursing program
Strongly Preferred Experience:
Prior experience in home health, care coordination, or a physician office setting
Work Environment:
Full-time, in-office position in the Case Management department, Monday-Friday, 8:00 AM-5:00 PM, working closely with the RN Case Manager, physician offices, and field staff to keep orders and documentation on track
