Private Duty Nurse LVN - Days (Part Time/Full Time) LiveWell Homecare Agency Round Rock, TX Job Details Part-time | Full-time 21 hours ago Qualifications CPR Certification BLS Certification Full Job Description We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, supervised by a Registered Nurse.

Responsibilities include:

Delivering and coordinating quality patient care according to physician orders Assessing patient condition, monitoring changes, and reporting as needed Documenting assessments, nursing actions, and care activities in a timely manner Implementing and updating care plans, recognizing patient needs, and preventing risks Educating patients and families on disease management, self-care, and prevention Ensuring infection control and maintaining confidentiality Qualifications and skills: Current, active LPN/LVN license in the practice state Current CPR certification TB test within the last 12 months

Preferred:

six months of recent clinical experience or home health background Strong attention to detail, organization, communication, and problem-solving skills Physical ability to lift 50 pounds, reposition patients, and perform home care tasks Reliable transportation and ability to travel Key duties: Providing compassionate care while adhering to policies and regulations Maintaining professional standards and documentation compliance Participating in ongoing education and skill development Adapting to varied home environments and potential exposure to health hazards
