Meaningful work driven by hearts We are seeking a compassionate and dependable Hospice Registered Nurse/ Licensed Vocational Nurse in San Angelo and surrounding areas to provide skilled nursing care to patients and families in the home setting. The RN/LVN will assess patient needs, manage symptoms, administer medications and treatments, provide education and support, and collaborate with the hospice interdisciplinary team to promote comfort, dignity, and quality of life.

• Assess and monitor hospice patients.
• Provide skilled nursing care and symptom management.
• Administer medications and treatments as ordered
• Educate and support patients and caregivers.
• Communicate changes in condition to the care team and physician
• Maintain accurate clinical documentation.
• Collaborate with the hospice interdisciplinary team.
• Promote patient comfort, dignity, and quality of life .

Qualifications Current, unrestricted RN/LVN license in the state of practice. Graduate of an accredited nursing program. Minimum of 1 year of nursing experience required.
