What you'll do as a clinical care coordinator (LVN) As a clinical care coordinator (LVN) , you keep our Sherman patients and clinicians in sync. Typically, your day includes: Scheduling and coordinating patient visits across the care team Reviewing clinical documentation and verifying physician orders Communicating with patients, families, and referral sources Supporting intake, authorizations, and care-plan updates Keeping records accurate, complete, and on time Moreover, you work closely with nurses and therapists while a supportive office team backs you up.
