Responsibilities Provide direct patient care by performing physical examinations, vital signs monitoring, and patient assessments with precision and compassion. Administer medications, injections, IV infusions, and other treatments following established protocols and safety guidelines. Assist with complex procedures such as catheterization, wound suturing, tube feeding, and sterile techniques to support optimal patient outcomes. Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner while maintaining HIPAA compliance. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse needs including dementia, Alzheimer's disease, or those requiring hospice or palliative care. Conduct triage assessments in urgent or emergency situations, utilizing critical thinking skills to prioritize care effectively. Support discharge planning and coordinate follow-up care to ensure seamless transitions for patients across settings such as inpatient units or outpatient clinics. Qualifications Valid Licensed Practical Nurse (LPN) license in good standing; additional certifications such as Basic Life Support (BLS) are required. Proven experience working in acute care environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, or emergency rooms. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Demonstrated knowledge of physiology, anatomy, ICD coding (ICD-9/ICD-10), medication administration, and medical terminology. Experience with specialized care areas including geriatrics, memory care (dementia/Alzheimer's), behavioral health, infectious disease management, or pediatric settings preferred. Ability to perform venipuncture, phlebotomy, sterile procedures, and IV insertion confidently while adhering to aseptic techniques. Strong communication skills with the ability to work effectively with individuals with disabilities or developmental challenges in diverse environments including assisted living or home & community care settings.
