This position manages clinical assessment schedules and care plans across assigned network buildings. You will rotate between facilities to verify ARD selection and align interdisciplinary documentation. The role works alongside facility Directors of Nursing to keep assessments on schedule.

Monday-Friday, 8PM - 5

Perform chart audits to verify clinical support for Medicare coding and PDPM classification. Review care plans with nursing leadership to maintain state compliance. Partner with therapy managers to align service days before billing submission. Lead triple-check pre-billing reviews to catch missing documentation. Provide hands-on coverage for assessments during staff transitions. Qualifications & Requirements Active Texas RN or LVN license in good standing Minimum 3 years of active MDS 3.0 and RAI process experience in skilled nursing RAC-CT certification preferred Working knowledge of PDPM coding and care planning Valid driver's license and willingness to travel between network facilities Must pass a standard criminal background check Must pass a drug screening Reliable transportation for consistent attendance Ready to Join Our Team?
