09/16/2026 Job Description Job Description Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our nursing team in a senior living and long term care environment. The ideal candidate is committed to providing person centered care to older adults while collaborating with an interdisciplinary healthcare team to promote residents' health, comfort, and well being. Key Responsibilities Provide direct nursing care in accordance with physician orders and individualized care plans. Administer medications, treatments, and therapies safely and accurately. Monitor residents' conditions and promptly report changes to the healthcare team. Perform routine assessments, document care, and maintain accurate medical records. Assist with admissions, discharges, and resident care planning. Collaborate with physicians, RNs, CNAs, therapists, and other healthcare professionals. Educate residents and families about care plans and health conditions. Ensure compliance with state and federal regulations as well as nursing standards. Promote a safe, respectful, and person centered environment. Qualifications Active Licensed Practical Nurse (LPN) license in good standing. Current CPR/BLS certification (preferred or required by state). Strong clinical assessment, communication, and documentation skills. Ability to work effectively in a collaborative healthcare environment. Compassionate, dependable, and committed to exceptional resident care. Preferred Qualifications 1-3 years of nursing experience in: Long Term Care (LTC) Skilled Nursing Facility (SNF) Assisted Living Rehabilitation Memory Care New graduates with an active LPN license are encouraged to apply.
