09/19/2026 Job Description Job Description Join our compassionate home care team and make a meaningful difference by providing dependable, high-quality nursing care in the comfort of the client's home. Key Responsibilities Provide nursing care according to the client's plan of care and RN direction. Administer medications and perform approved treatments and procedures. Monitor the client's condition and promptly report any changes. Complete accurate and timely clinical documentation. Follow infection-control and safety procedures. Educate and support clients and their families. Communicate client concerns and care updates to the supervising RN. Protect client privacy and follow HIPAA requirements. Participate in training and quality-improvement activities. Qualifications Active Virginia or Compact LPN license. At least one year of nursing experience preferred. Current CPR/BLS certification. Experience caring for clients with a tracheostomy and G-tube. Home care or community health experience is a plus. Ability to recognize and report changes in a client's condition. Strong communication, documentation, and organizational skills. Compassionate, dependable, and attentive to client needs. Preferred Skills Clear verbal and written communication. Active listening and careful observation. Strong teamwork and coordination. Service-focused and respectful approach. Ability to work independently in a home care setting.
