Job Description This position is located at 1825 & 1829 Rokeby Ave, Chesapeake, VA. Are you a compassionate Licensed Practical Nurse who believes quality healthcare begins with meaningful relationships? At Chesapeake Integrated Behavioral Healthcare (CIBH) we support and help individuals with intellectual and developmental disabilities live healthier, fuller, and more independent lives. As an LPN in our 24-hour Intermediate Care Facility (ICF), you'll be part of an interdisciplinary team dedicated to providing exceptional nursing care in a supportive, person-centered environment. This is more than a nursing job—it's an opportunity to positively impact lives every day. What You'll Do Provide direct nursing care in accordance with physician orders and individualized care plans. Administer medications and treatments while ensuring accuracy and compliance with nursing standards. Monitor residents' physical and emotional health, documenting observations and reporting changes promptly. Collaborate with physicians, registered nurses, therapists, direct support professionals, and other members of the interdisciplinary team. Assist with medical appointments, health assessments, and wellness initiatives. Maintain accurate medical records and ensure compliance with state, federal, and DBH regulations. Promote a safe, respectful, and therapeutic living environment for all residents. Participate in emergency response and provide nursing guidance to direct care staff.

SPECIAL CERTIFICATIONS AND LICENSES
Requires licensure as a Licensed Practical Nurse by the Commonwealth of Virginia or a reciprocal state (according to the Nurse Licensure Compact as prescribed by the National Council of State Boards of Nursing). Requires certification in CPR, Multi-media First Aid, and Handle With Care. May require a valid driver's license and a driving record that is in compliance with the.
