Job Description

Health Advocates Network, Inc. is seeking a compassionate LPN to deliver high-quality patient care across diverse clinical settings. In this role, you'll provide hands-on nursing care, administer medications and treatments, monitor and document patient status, and support care plans under the direction of RNs and physicians. You will collaborate with multidisciplinary teams, educate patients and families, and uphold rigorous safety and infection control standards. Responsibilities Provide direct bedside nursing care under RN/physician direction Administer medications and treatments safely and accurately Monitor, document, and report patient status and vital signs Perform wound care, dressing changes, and basic clinical procedures Update and maintain accurate EMR documentation Educate patients and families on treatments and discharge instructions Collaborate with interdisciplinary healthcare teams for coordinated care Follow infection control, safety, and regulatory guidelines Support continuity of care across multiple facility assignments Required Skills Patient assessment Vital signs monitoring Medication administration Wound care and dressing changes Care plan implementation Electronic Medical Records (EMR) documentation IV therapy and injections Infection control practices Patient and family education Collaboration with RNs and physicians
