Qualifications:

High school diploma or equivalent

License/Certification:

Virginia State Nursing required Current CPR/BLS required At least 2 years LPN experience required ; Cardiology experience preferred Criminal background clearance Must be available to work from 8am-5pm Mon-Thurs, and 8am-12pm Fri, no weekends.

Responsibilities:

The Licensed Practical Nurse (LPN) functions under the supervision of NP, or MD in a medical office setting, within the scope of practice dictated by the State Board of Nursing. LPN works as part of a multi-disciplinary team to support and communicate effectively with Physicians & NP in collaboration while providing care and treatment for patients, clinical support and teaching for patient and family members, and adheres to the legal and ethical standards of nursing practice. LPN manages responsibilities of workload with accuracy, provides excellent customer service, performs testing (includes stress testing, heart monitor hook-ups), refills prescription and obtain prescription prior authorizations, scan and upload clinical documentation to patient EHR, Coumadin management with orders from Physician and NP, and coordinate referrals for Watchman Procedure, LifeVest, Leqvio, & Lymphedema Pumps. LPN will cover for MA, includes patient triage; to include vital signs, EKG, medication reconciliation, and obtain detailed patient history. LPN implements care plans for

CCM & RPM

of patients with HF, HTN, and CAD; serves as patient liaison between patients and Physician & NP. Communication with vendor to enroll patients & order RPM equipment. LPN communicates test results to patient by phone.

Summary:
