Job Description

Care Advantage, Inc. is looking for multiple Licensed Practical Nurses (LPN) to support a weekend case in Prince George, VA area. Shifts would run from 12 PM to 8 PM. Our care teams provide one-on-one care in clients' homes to provide the best care possible to our patients. Our goal is to decrease depression and mitigate cognitive impairment with specialized care plans developed by our Registered Nurses while keeping our patients safe and comfortable in their own homes. LPN Responsibilities:
Provide and implement direct patient care Provide accurate and timely documentation consistent with the plan of care Assess and provide family/caregiver education and information Assist the RN or physician in performing specialized procedures and duties, which include preparing equipment and materials for treatments, observing aseptic technique as required, and assisting the patient in learning appropriate self-care techniques Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or
Clinical Supervisor Requirement/Eligibility:
Eligible to work in the United States CPR Certification and PPD Certification 1+ year experience as an LPN Valid LPN License with at least (1) one year of work experience Valid Driver's license Reliable transportation Experience with trachs, vents and peg tubes Ability to work in the home with pets Care Advantage, Inc. and its affiliates are Equal Opportunity/Affirmative Action employers.
