Job Description
1st Americare is seeking an LPN to work with our client in the Vienna area. Candidate with Gtube, Vent, & Trach experience. Must be able to work all three days. The schedule for this case will be: Friday, Saturdays, & Sundays 7:30pm-7:30am
Duties and Responsibilities:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans accordingly
- Educate patients and their families on managing their conditions and promoting wellness
- Collaborate with other healthcare professionals to coordinate patient care
- Document patient care activities accurately and timely
