BrightStar Care of Chesapeake/Virginia Beach/Williamsburg is seeking an experienced and dependable Licensed Practical Nurse (LPN) for a potential private duty nursing case requiring tracheostomy and ventilator care . This is an overnight position with a consistent Sunday through Thursday schedule from 10:00 PM to 6:00 AM . Because of the clinical needs of this client, previous hands-on experience caring for patients with both tracheostomies and mechanical ventilation is required. We are specifically looking for an LPN who is comfortable independently managing a trach/vent patient in the home setting and responding appropriately to changes in the patient's condition. Schedule Sunday through Thursday 10:00 PM - 6:00 AM 40 hours per week Consistent overnight schedule One-on-one private duty nursing Responsibilities Provide one-on-one skilled nursing care in the patient's home according to the established Plan of Care and physician orders. Provide ongoing tracheostomy and ventilator care and monitoring. Monitor respiratory status, ventilator function, oxygenation, and overall patient condition throughout the shift. Perform tracheostomy care and suctioning as required. Recognize and respond appropriately to respiratory distress, ventilator alarms, airway complications, and other changes in condition. Administer medications, treatments, and other skilled nursing services as ordered. Monitor vital signs and document assessments, interventions, treatments, and patient response. Maintain airway safety and ensure respiratory equipment and emergency supplies are readily available. Follow infection control procedures and established clinical protocols. Communicate changes in condition, concerns, and significant events promptly to the Director of Nursing and other appropriate members of the healthcare team. Maintain accurate and timely documentation using the agency's electronic medical record (EMR) system. Provide compassionate, professional care while maintaining the patient's dignity, safety, and comfort. Qualifications Current Virginia LPN license in good standing. Previous hands-on tracheostomy care experience required. Previous hands-on ventilator patient experience required. Must be comfortable providing trach and ventilator care independently during an overnight shift. Private duty nursing, home health, pediatric, pulmonary, respiratory, ICU, LTAC, or other relevant high-acuity experience strongly preferred. Strong clinical observation and assessment skills. Ability to recognize changes in respiratory status and respond appropriately to urgent situations. Current CPR certification. Comfortable utilizing electronic charting and documentation systems. Must be able to pass a background check with no barrier crimes or theft-related offenses. Must be able to pass a 10-panel drug screen. Negative TB test within the past year. Reliable transportation.
