LPN - Adult Home Health UVM Home Health & Hospice is seeking a compassionate and dedicated LPN to join our growing Adult Home Health team! In this role, you'll play a vital part in helping patients receive high-quality care in the comfort of their own homes. What you'll do: Provide skilled nursing care under the supervision of an RN, following established plans of care. Assess patients' physical, emotional, and social needs. Administer medications, wound care, and other treatments. Educate patients and families on healthcare needs and self-management strategies. Collaborate with other healthcare professionals to ensure comprehensive care.
What you bring:
Active LPN license in Vermont. Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience Strong assessment, communication, and interpersonal skills. Compassionate and patient-centered approach. Excellent teamwork and collaboration abilities.
We offer:
$7,500 Sign on bonus eligible for full time positions, additional terms and conditions apply. $9,200 relocation package eligible for full time positions. Competitive salary and benefits package.
