Job Description

The University of Vermont Health Network seeks a Practice LPN to support patient-centered care in an outpatient clinic. In this role, you'll provide direct clinical care, obtain histories and vital signs, administer medications and immunizations, and assist with exams and minor procedures. You'll use EMR tools to document accurately, coordinate patient flow, and support education for diverse patients across Vermont and northern New York. Working in a collaborative, academic environment, you'll partner with nurses, providers, and specialists, apply evidence-based practices, and grow professionally through ongoing learning and quality initiatives. Responsibilities Provide direct patient care under RN and provider direction in an outpatient practice Obtain vital signs, patient histories, and perform basic clinical procedures Administer medications and immunizations per protocol and document accurately Assist with exams, treatments, and minor office procedures Coordinate patient flow, room patients, and prepare exam rooms Support patient education, discharge instructions, and care coordination Maintain infection control, safety, and quality standards Use EMR systems for charting, orders, and scheduling Collaborate with interdisciplinary teams to support evidence-based care Participate in continuous learning, quality improvement, and competency training Required Skills Patient assessment and vital sign monitoring Medication administration and immunizations Outpatient clinical procedures (EKG, phlebotomy, wound care) Electronic Medical Record (EMR) documentation Infection control and patient safety practices Care coordination and patient flow management Patient education and discharge teaching Triage and telephone encounter documentation Team-based, interdisciplinary collaboration Time management and prioritization in clinical setting
