Responsibilities Deliver comprehensive nursing care to patients across multiple healthcare settings, including acute, long-term, and community-based environments. Perform patient assessments, vital signs monitoring, and physical examinations with precision and attention to detail. Administer medications, injections, IV infusions, and perform catheterizations following strict protocols and safety standards. Manage complex cases such as dementia care, Alzheimer's care, hospice support, pain management, and infectious disease protocols. Document all patient interactions accurately using Electronic Health Records (EHR) systems like Epic, Cerner, or Athenahealth; ensure compliance with HIPAA confidentiality policies. Collaborate effectively with interdisciplinary teams to develop and implement personalized care plans tailored to individual needs. Provide education and support to patients and their families on health management, medication administration, and discharge instructions. Requirements Valid LPN license in good standing with current state licensure. Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; experience with ICD coding (ICD-9/10) is a plus. Strong background in medical-surgical nursing, geriatrics, long-term care, hospice care, or home health is preferred. Knowledge of advanced nursing procedures such as IV insertion/infusion, tube feeding management, sterile technique, and airway management. Experience working with diverse patient populations including pediatric (PICU), neonatal ventilator management, or individuals with disabilities or developmental delays. Excellent communication skills for caregiver interaction and client education; ability to adapt quickly to new environments and protocols.
