Memory Care LPN to be an integral team player and leader working with a dynamic, fast paced Memory Care Team.

Job Overview We are seeking a passionate and energetic Memory Care Licensed Practical Nurse (LPN) to lead our memory care program with compassion and expertise. In this vital role, you will oversee the delivery of specialized care for residents with dementia and memory impairments, ensuring they receive personalized support that promotes dignity and quality of life. Your leadership will inspire our team to provide exceptional patient care while maintaining compliance with healthcare regulations. Responsibilities Develop and implement individualized care plans tailored to residents with memory care needs, including Alzheimer's and other dementias Supervise care staff, providing guidance on clinical best practices and fostering a collaborative team environment Conduct comprehensive assessments of residents' physical, emotional, and cognitive health using medical terminology and physiology knowledge Ensure adherence to HIPAA regulations and maintain accurate medical records using EMR (Electronic Medical Records) systems. Administer medications, injections, tube feedings, IV infusions, and manage ventilator or catheter care as needed with sterile technique and aseptic procedures Oversee compliance with state healthcare regulations, long-term care regulations. Coordinate with families for discharge planning, family meetings, and behavioral health support while ensuring compassionate communication Requirements Valid LPN license with current state licensure; experience working in long-term care or assisted living environments preferred Demonstrated experience in memory care or Alzheimer's/dementia-specific settings Proficiency with EMR/EHR systems.
