Responsibilities Deliver direct patient care by monitoring vital signs, administering medications, and performing clinical assessments with precision and compassion. Assist with complex procedures such as IV infusion, catheterization, wound care, and sterile techniques following established protocols. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Maintain accurate and detailed medical documentation using Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and long-term care regulations. Conduct patient triage over the phone or in person, assessing urgency levels for emergency room or urgent care situations. Support discharge planning by educating patients and families on post-care instructions, medication management, and follow-up appointments. Ensure infection control standards are upheld through proper hand hygiene, sterilization procedures, and adherence to safety policies. Skills Proven experience working with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities, dementia or Alzheimer's disease, and those requiring hospice or memory care. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques. Skilled in performing physical examinations, specimen collection/processing, venipuncture, phlebotomy, suturing, and airway management. Proficiency with EMR/EHR systems such as Epic, Cerner or Athenahealth; experience managing electronic health records efficiently. Ability to handle acute pain management cases in settings like ICU or Level I Trauma Centers; experience in emergency medicine or hospital medicine is highly valued. Excellent caregiver communication skills to effectively educate patients and coordinate with healthcare teams across settings including skilled nursing facilities and home & community care environments. Knowledge of infection control practices, HIPAA confidentiality policies, first aid procedures, and long-term care regulations.
