We are highly patient oriented and are looking for a very kind, nurturing and enthusiastic LPN for our high level care residents! We are special in that our nurse to resident ratio is 1:2. We have 3 staff members on during the day to help care for just 6 residents. We have a wonderful staff and are looking for just 1 more member to complete our team! CPR Certification (Preferred) We are looking for a Licensed Practical Nurse to join our team and provide quality care to our patients. The ideal candidate will have experience in long-term care, home health, wound care, and other areas of nursing. The successful candidate will be responsible for monitoring patient health, providing treatments, administering medications, and educating patients and their families on health maintenance and disease prevention.

Responsibilities:

Provide direct patient care in accordance with the state Nurse Practice Act. Document patient care and progress in the medical record. Provide long-term care to patients in a home health setting. Administer medications and treatments as prescribed by the physician. Assess and monitor patient's condition, including vital signs, intake and output, and wound care. Provide education to patients and families regarding health maintenance and disease prevention. Assist with activities of daily living.
