Position Summary:
We are seeking an experienced Licensed Practical Nurse (LPN) to join a rehabilitation healthcare team. The LPN will provide direct patient care to individuals recovering from illness, injury, surgery, or other medical conditions while supporting their recovery and rehabilitation goals.
Key Responsibilities:
Provide direct nursing care to rehabilitation patients according to individualized care plans. Monitor patients' conditions and report changes to the appropriate healthcare provider. Administer medications and treatments as ordered. Perform routine assessments and document patient care accurately. Assist patients with activities of daily living and mobility as needed. Monitor vital signs and observe patients for changes in condition. Collaborate with RNs, physicians, therapists, and other members of the healthcare team. Educate patients and families regarding medications, treatment plans, and ongoing care. Maintain accurate and timely clinical documentation. Follow facility policies, infection-control procedures, and patient-safety standards.
Requirements:
Current and active LPN/LVN license. Previous LPN experience; rehabilitation experience required. Current BLS certification. Strong clinical assessment and communication skills. Ability to work effectively in a fast-paced healthcare environment. Must be able to work the assigned schedule. Must meet all facility-specific credentialing and compliance requirements.
