Position Summary We're seeking a skilled and mission-driven (Pediatric) Licensed Practical Nurse to deliver direct patient care, coordinate services, and support our community health initiatives. This role blends clinical expertise with compassionate outreach, ensuring patients receive comprehensive, culturally sensitive care. Key Responsibilities Conduct thorough patient assessments using subjective and objective data Administer care per established protocols under the direction of the Medical Director Develop individualized care plans, including education and referrals Refer patients to appropriate medical and human service agencies using knowledge of community resources Provide health education and counseling to support patient wellness Report communicable diseases in compliance with federal, state, and local regulations Perform venipuncture and administer parenteral medications and nebulizer treatments as ordered Accurately document all patient interactions and treatments Uphold legal and ethical standards in nursing practice Apply knowledge of the Wisconsin Nurse Practice Act and scope of practice Adapt to evolving trends in nursing and demonstrate clinical competence Utilize and document appropriately within grant-funded programs (e.g., adult/children's medication, eyeglass, dental, diabetic programs) Coordinate care with volunteer staff and interdisciplinary teams Maintain compliance with OSHA standards and clinic safety protocols Qualifications Current RN license in the State of Wisconsin Strong clinical assessment and documentation skills Experience in community health or public health preferred Knowledge of local support services and referral networks Excellent communication and patient education abilities Commitment to health equity and serving diverse populations Pediatric care experience preferred Why Join Us?
