From $1,600/week Position Summary We are seeking a reliable and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and rehabilitation setting . This role involves delivering compassionate, resident-centered care , supporting clinical operations, and collaborating with interdisciplinary teams to ensure optimal patient outcomes . The ideal candidate is flexible and comfortable working day and evening shifts . Key Responsibilities Patient Care & Treatment Administer medications and treatments as prescribed Provide direct nursing care while ensuring resident safety and comfort Monitor resident conditions and report changes to the care team Clinical Support & Documentation Assist with: Resident assessments Care planning Clinical documentation Maintain accurate and timely patient records Team Collaboration Work closely with: Certified Nursing Assistants (CNAs) Nursing staff Interdisciplinary care teams Support coordinated and efficient care delivery Requirements Active Licensed Practical Nurse (LPN) license (required) Experience in: Long-Term Care (LTC) or Skilled Nursing Facility (SNF) (preferred) Ability to work: Rotating day and evening shifts Every other weekend Flexibility to work 5 shifts per week as needed Additional Details Overtime opportunities available Flexible scheduling options, including: Two 16-hour shifts + one 8-hour shift (where applicable) Supportive team environment focused on quality care and resident well-being For more details reach at vgill@navitassols.com or Call / Text at 516-862-1203 .
