Job Description
Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this role, you will provide high-quality nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and personalized care plans. The ideal candidate will demonstrate strong clinical skills, excellent communication abilities, and a commitment to delivering respectful, patient-centered care. Duties Conduct thorough patient assessments by monitoring vital signs, evaluating physical conditions, and documenting findings accurately within electronic health record (EHR) systems. Administer medications and injections following established protocols, including IV infusions and catheter care. Assist with diagnostic evaluations such as specimen collection, laboratory processing, and medical imaging preparations. Support patients with activities of daily living (ADLs), including mobility assistance, tube feeding, wound care, and hygiene management. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to patient needs. Maintain compliance with HIPAA regulations and clinical confidentiality policies while managing medical records. Provide emergency response interventions such as first aid or airway management when necessary. Educate patients and their families on health management strategies, medication use, and disease-specific care such as dementia or Alzheimer's. Ensure infection control practices are followed rigorously to maintain a safe environment for patients and staff. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). Proven experience working in diverse healthcare settings such as hospitals (including ICU or trauma centers), nursing homes, hospice care, or outpatient clinics. Knowledge of EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Familiarity with medical terminology, ICD coding (ICD-9/10), and medication administration protocols. Experience with specialized care areas including geriatrics, memory care (dementia/Alzheimer's), acute pain management, infectious disease care, and behavioral health. Ability to perform clinical procedures such as venipuncture, catheterization, sterile techniques, suturing assistance, and airway management. Strong communication skills for effective caregiver-patient interactions and team collaboration. Basic life support (BLS) certification; additional certifications such as IV insertion or advanced life support are advantageous. Demonstrated understanding of long-term care regulations and patient assessment standards. Flexibility to work in various environments including inpatient units, outpatient clinics, home & community care settings, or travel nursing assignments.
