Evenings with the occasional Day shift available. Schedule will consist of EOW. Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will play a vital role in delivering high-quality nursing services, supporting patient recovery, and ensuring compliance with healthcare regulations. Your commitment to providing exceptional care will help improve the health and well-being of our diverse patient population. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and documentation of medical histories using Electronic Health Records (EHR) systems. Administer medications and injections accurately following physician orders and established protocols, including IV infusions and catheter care. Provide wound care, including suturing, dressing changes, and sterile technique adherence to prevent infection. Support patients with activities of daily living (ADLs), including mobility assistance with Hoyer lifts and tube feeding management. Educate patients and their families on health management strategies, medication administration, and discharge planning while maintaining clinical confidentiality policies. Collaborate with interdisciplinary teams across specialties such as geriatrics, pediatrics, behavioral health, and hospice care to ensure comprehensive patient support. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience working in diverse healthcare environments including hospitals, nursing homes, skilled nursing facilities, or home care settings. Strong understanding of medical terminology, anatomy knowledge, and physiology to perform accurate patient assessments. Experience working with individuals with disabilities or developmental disorders is highly desirable. Ability to handle emergency situations effectively using CPR/Basic Life Support (BLS) skills in accordance with safety protocols. Excellent communication skills for caregiver communication and client education within a multidisciplinary team environment.
