Company Overview Grant Gardens Residential Treatment Center provides a structured, supportive environment for adolescents facing emotional and behavioral challenges. Through evidence-based treatment, psychiatric services, and coordinated care, we help residents build healthy coping skills, strengthen relationships, and prepare for a successful return to their communities. Job Overview We are seeking one dedicated and compassionate Part-Time Licensed Practical Nurse (LPN) to join our multidisciplinary team. At Grant Gardens, our nurses build trusting relationships with residents, support their physical and emotional well-being, and help create a safe environment for treatment and growth. Working under the direction of nursing leadership, you'll provide practical nursing care, administer medications, monitor residents' responses to treatment, and communicate changes in their condition. You'll collaborate with nursing, clinical, and residential staff to support each resident's individualized treatment plan. Responsibilities Provide nursing care to adolescent residents within the LPN scope of practice and under the direction of nursing leadership. Collect and document health information, obtain vital signs, monitor physical and behavioral symptoms, and promptly report changes to the appropriate nurse or provider. Administer prescribed medications and treatments accurately; monitor therapeutic effects, side effects, and adverse reactions. Maintain accurate medication administration records and follow procedures for controlled medication storage, counts, documentation, and discrepancy reporting. Participate in individualized treatment planning and implement assigned nursing interventions in collaboration with the treatment team. Support a safe, therapeutic environment through respectful communication, professional boundaries, trauma-informed care, and approved de-escalation techniques. Recognize and promptly escalate medical or psychiatric emergencies and assist with emergency response according to training and facility procedures. Provide resident and family education on medications, health conditions, hygiene, nutrition, and other wellness topics. Complete accurate and timely nursing documentation in the facility's designated electronic health record system. Support admission and discharge processes, including resident orientation, health information collection, and communication of ongoing care needs. Follow infection prevention, resident safety, confidentiality, and facility policies. Participate in required training, nursing meetings, and quality-improvement activities. Experience Graduate of an accredited practical nursing program required. Current LPN license authorizing practice in West Virginia required. Experience in behavioral health, psychiatric nursing, residential treatment, or adolescent care preferred, but not required. Knowledge of medication administration, medication effects and contraindications, infection prevention, and practical nursing procedures. Strong observation, documentation, communication, organization, and teamwork skills. Ability to remain calm, prioritize responsibilities, and respond appropriately during stressful situations. Comfortable using electronic health records and learning facility documentation systems. Ability to establish appropriate therapeutic relationships with adolescents while maintaining professional boundaries. Must obtain and maintain CPR, First Aid, AED, and applicable facility-required crisis intervention certifications. Must complete required background clearances, health screenings, and ongoing facility training.
