Roane General Hospital in Spencer, WV a Critical Access Hospital with 35 SNF/ICF beds, 25 acute care beds, Level IV Trauma Emergency Department, three Rural Health Clinics and a walk-in clinic is currently seeking applicants for a Full-Time Case Management LPN at Roane General Medical Clinic.

• $2500 SIGN

• Qualifications:

• Current Licensed Practical Nurse license required.
• Previous experience in caring for chronic disease patients required.
• Experience in clinical or community health settings preferred.
• Previous Care Coordination or Case Management preferred.
• Demonstrates evidence of essential leadership, communication, education, collaboration, and counseling skills.
• Proficient in various communication technologies.
• Effective organizational skills and ability to maintain accurate notes and records.
• Previous experience with health IT systems and data reports preferred.
• Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with a variety of populations preferred.
• Ability to identify and implement appropriate patient communication strategies and overcome accessibility barriers, as required.
• Orchestrate the effective delivery of care in accordance with third-party payer program requirements and resources.

Coordinates multidisciplinary care to provide health services to individuals, through effective partnerships with patients, caregivers, families, community resources, and their physician. Facilitates navigation within a patient centered model and across settings to achieve coordinated care that is efficiently provided in an optimal manner. The ability to multitask and work in a fast-paced clinic setting is essential.
