A Licensed Practical Nurse or Vocational Nurse provides basic medical care under the direction of registered nurses and other healthcare specialists. The tasks that they are permitted to perform vary by state, but usually include changing bandages, checking blood pressure, or helping feed patients. May work in hospitals, nursing homes, or other healthcare facilities.

We are currently seeking a reliable, skilled, and compassionate Licensed Practical Nurse (LPN) to join our growing team. In this role, you will deliver essential, client-centered nursing care directly in the comfort of our clients' homes, helping them maintain their independence, dignity, and quality of life.
Responsibilities:
Provide direct, one-on-one nursing care in accordance with individualized care plans and physician orders. Monitor, record, and report vital signs, symptoms, and changes in clients' physical or mental condition. Administer prescribed medications, treatments, and wound care in compliance with state regulations. Collaborate closely with clients, family members, physicians, and our care team to ensure seamless continuity of care. Maintain accurate, timely, and complete clinical documentation. Educate clients and their families on health maintenance, disease management, and safety protocols.
Qualifications & Requirements:
License:
Current, unencumbered Licensed Practical Nurse (LPN) license in the State of Wyoming (or compact state).
Certifications:
Active
CPR / BLS
Certification (required).
Experience:
Minimum of 1 year of clinical nursing experience preferred (home health, long-term care, or geriatric care experience is a plus).
Transportation:
Valid Driver's License, reliable transportation, and auto insurance.
Skills:
Strong clinical judgment, excellent communication skills, and a warm, patient-first approach to care.
Background Check:
Ability to pass a standard criminal background check.
