Position Summary The Certified Medical Assistant supports the CCM/RPM team by assisting with patient enrollment, outreach, device setup, and administrative functions that ensure smooth program operations. The CMA plays a vital role in helping patients stay connected, engaged, and supported in their chronic care journey. Key Responsibilities Identify and enroll eligible patients into CCM and RPM programs. Assist patients with device education, connectivity, and troubleshooting. Perform routine outreach to collect updates on patient progress and adherence. Update and maintain patient data within the EHR and monitoring platforms. Communicate relevant updates or concerns to the supervising nurse coach. Support patient education materials and care plan documentation. Participate in team huddles and contribute to quality improvement initiatives. Qualifications Certified Medical Assistant (AAMA, AMT, or equivalent). 2 years experience with

CCM & RPM

Minimum 1 year of experience in a clinical or patient outreach setting. Proficient with eeClinical Strong interpersonal and organizational skills. Interest in chronic disease prevention and management.
