HarmonyCares is seeking a Medical Assistant Clinical Partner to support our home-based primary and palliative care teams. In this field-based role, you'll assist with patient assessments, obtain vitals, perform basic clinical tasks, and document in the EHR during in-home and telehealth encounters. You'll coordinate visits, support care plans, reinforce patient education, and communicate changes in condition to the clinical team. Working closely with physicians, nurses, and social workers, you help reduce avoidable hospitalizations and improve access, safety, and comfort for medically complex, home-limited patients.
