About Coal Country Community Health Center Coal Country Community Health Center (CCCHC) is celebrating more than 20 years of caring for the community with four health center locations, and five school-based clinics serving the communities of Beulah, Hazen, Center, and Killdeer in West Central North Dakota. As a Federally Qualified Health Center, we care for patients of all ages, focusing on quality primary care, behavioral health, and wellness for your family, CCCHC cares for over 9,000 patients annually.
