A remote care platform is created by clinicians, for clinicians. We are seeking experienced Certified Medical Assistants who have Chronic Care Management and /or Remote Patient Monitoring experience to add to our work team in Mohegan Lake, NY. About this position Under the general supervision of the provider and HealthArc Care Team Lead , and working in conjunction with the patient's Care Team, the Care Coordinator is responsible for the provision of safe therapeutic and efficient care and services to patients, supporting a patient panel through providing synchronous and asynchronous communications and managing the coordination of care. Functions as a member of the HealthArc Care Team to assist in the planning and implementation of patient care based upon sound clinical preparation, knowledge, skills, and experience. Care is provided in accordance with the provisions of Medical Assistant Certification . Essential Job Functions Conducts interviews and questionnaires for eligible patients. Interacts and communicates synchronously and asynchronously in person, via telephone, video, email or text. Serves as the primary point of contact for the patient participating in any Care Management program; manages care and services and ensures effective communication among primary care providers, caregivers, providers and community supports Assists in the creation, review, revision and maintenance of an up-to-date comprehensive care plan for each patient. Utilizes SMART goal format to assist patients with setting and attaining their specific goals Utilizes motivational interviewing and evidence based approaches to empower patients to take charge of their own wellness in the context of their care plan and to influence patient behavioral changes to increase the level of compliance with customized care.. Works closely with the HealthArc Care Team to manage the day to day communications involving: symptom control, medication management, coordination of care and provide patient and family education Assists patients with use of devices as related to remote physiologic or therapeutic monitoring Communicates with physician, Nurse Practitioner, RN, Social Worker, Pharmacist or other health care providers regarding patient's status Collaborates with the patient's care team when the patient requires intervention or follow-up. Attends and prepares patient discussion for team huddles Manages case load, including monitoring reassessment needs and facilitating transitions of care. Maintains timely, complete and accurate documentation using HealthArc technology and electronic medical records. All other duties as assigned.
Education/Experience:
Certified Medical Assistant with active certification; minimum 2 years clinical experience with focus in care management or population health or health and wellness coaching.
Skills, Knowledge & Abilities:
Knowledge of and proficiency in: (1) providing care for patients with chronic illness, and proven commitment to excellence in patient care; (2) using the principles of care management and person-centered service planning; (3) familiar with clinical needs for chronic illnesses (diabetes, heart disease, hypertension, asthma, dementia, etc.); (4) establishing priorities, knowing when to escalate patient issues and completing multiple projects on time, and within budget; (5) hands-on proficiency with electronic health records (EHR) software; (7) knowledge of ICD-10 (10th revision of the International Statistical Classification of Diseases and Related Health Problems) as published by the World Health Organization; (8) Understands and adapts appropriately to issues related to communication, cognitive or other barriers.
Communications:
Demonstrated effective and diplomatic oral and written communication skills, including telephone, video, text and email. Demonstrated experience using diplomacy to negotiate, to resolve conflicts, and to persuade others to embrace strategies proposed. Ability to effectively present clinical information to the care team; Proficient in navigating and entering information into an electronic health record; models team work; Self-motivated and dependable; Able to work independently.
Licensure and Certification:
Two (2) or more years healthcare experience as a Certified/Registered Medical Assistant with active certification, including demonstrated proficiency with multiple EMR/EHR systems. Demonstrated proficiency with Microsoft Office and related technologies.
