Opens a new window. Description The Dementia Clinic Assistant will collaborate with the Dementia Clinical Pharmacist to support the Dementia Clinic. This role will function as a non-licensed specialist, providing expertise in resources, care coordination, and escalation to the clinician for members with Alzheimer's dementia. Professional Duties Contact patients and patients' families to follow up on the Clinical Pharmacist's recommendations. Document patient and family interactions in the electronic medical record under the minimum supervision of the Clinical Pharmacist. Oversees lower acuity level patient needs by performing needs assessment and applying program interventions. Monitors and interprets patients' and families' needs by identifying families in crisis and follows protocols to ensure that their needs are met. Communicates professionally and compassionately via telephone and via email. Makes referral to appropriate community-based services and applies non-pharmacologic management of dementia-related behaviors and caregiver education/support. Meets weekly with the Clinical Pharmacist to summarize interventions instituted for these patients. Works collaboratively with the Clinical Pharmacist to ensure that patient acuity assignments are accurate and up-to-date. Perform other duties as assigned. Qualifications Associate's degree required. Bachelor's degree preferred. 1 year of experience in a healthcare setting required. 2 years of experience with dementia-related programs strongly preferred. Working knowledge of human biological systems. Strong interpersonal and communication skills, with the ability to engage passionately with patients and families. At the discretion of
