Benefits Bonus based on performance Competitive salary Dental insurance Opportunity for advancement Paid time off About the Role Woodlands Primary Healthcare is a growing primary care practice seeking a detail-oriented, self‑motivated Medical Assistant to support our Chronic Care Management (CCM) and Principal Care Management (PCM) programs remotely. This is an excellent opportunity for a bilingual MA who thrives in a patient-centered environment and is comfortable working independently from home. You will serve as a key touchpoint for our chronic disease patients — conducting monthly care coordination calls, documenting clinical interactions, updating care plans, and ensuring patients remain engaged with their treatment goals between office visits. This is a remote position requiring reliable internet, a quiet workspace, and strong self‑discipline. Key Responsibilities Conduct monthly CCM/PCM outreach calls with enrolled patients to review symptoms, medications, and care plan progress Accurately document all encounters and time logs in eClinicalWorks (ECW) Coordinate care between specialists, PCP, and ancillary providers Assist with care plan creation and updates under provider supervision Identify and elevate urgent patient concerns to the clinical team Maintain compliance with CMS billing requirements for CCM/PCM time tracking Support patient enrollment and consent documentation Requirements Medical Assistant certification (CMA, RMA, or equivalent) preferred Minimum 2-3 years of experience in a clinical or care coordination role Proficiency in eClinicalWorks (ECW) — required, no exceptions Experience with CCM, PCM, or RPM programs strongly preferred Bilingual — English/Spanish required (spoken and written) Must reside in the Spring, TX or Katy, TX area Reliable home office setup with stable internet connection Strong communication, documentation, and time‑management skills Ability to work independently with minimal supervision
Compensation & Schedule Pay:
