Careteam Plus provides accessible primary and specialty healthcare services across the Grand Strand region. Our clinics serve patients of all ages, focusing on continuity of care, accessibility, and high-quality outcomes. We are committed to reducing barriers to healthcare and delivering reliable medical services to individuals and families, regardless of ability to pay and insurance coverage. Position Overview Under required general supervision, the Chart Prep & Quality Certified Medical Assistant (CPQ-CMA) reviews, organizes, and completes patient records before clinical appointments to ensure healthcare providers and team have advance accurate information. Clearly understands quality key performance indicators for clinical area and identifies patient care gaps. Assists providers in identifying opportunities to close care gaps and overall improve patient care. Responsibilities may include verifying patient demographics, update medical histories, and compile necessary documents. This role helps streamline workflow, reduce errors and improve patient care. Works very closely with providers, LPN's, CMA's and front desk operations teams to improve and deliver patient care. Additionally, provides direct patient care services occasionally/on an as needed basis when staffing levels call for team assistance. When that occurs, provides patient care by assisting with the collection of patient health data to be used to complete a physical assessment, administering treatment and medications and performing diagnostic tests ordered. Accurately records patient medical history, performs medication reconciliation, provides patient education and facilitates care coordination and continuity of care acting as a liaison with other team members. Key Responsibilities Chart Prep Responsible for reviewing all charts prior to appointments to ensure that all medical records are on file Collecting necessary forms, documents, authorizations and patient medical records to ensure a smooth experience before and at the beginning of appointments. Check upcoming appointment schedules and verify all necessary records, test results, and reports are on file. Confirm patient demographics, insurance authorizations, and contact information. Request missing documentation or outside medical records from other facilities. Update electronic health record (EHR/EMR) systems and ensure accurate dates of service. Coordinate with providers, nurses, and administrative staff to resolve missing information and support patient flow. Communicate as needed with providers and other health care professionals to obtain additional information. Submit files/documentation to providers and others as requested for review, quality assurance checks, and more. Obtain records from outside sources for preview prior to the office visit Utilize health information platforms from across the region (record-sharing applications). Index, and accurately file documents within the EMR. Complete additional related administrative tasks, as needed Direct Patient Service Performs health screenings as ordered by Providers. Verifies patient info by interviewing patients, recording medical history, confirming/verifying reason for visit. Prepares patients for examination by performing preliminary physical tests, taking vitals and reporting patient history summary. Collaborates with provider to arrange for medical treatment and complete referrals as ordered in accordance with established policies and procedures. Conducts appropriate CLIA Waived laboratory tests and quality control procedures as ordered by providers. Documents accurately elements of patient care in electronic medical record including but not limited to communication, treatments, immunizations, medications administered, discharge instructions provided & follow up care arranged. Administers medications and medical treatments competently as directed by a provider. Maintains a clean and safe environment. Follows infection control procedures. Maintains patient privacy and confidential patient information in accordance with established policies and procedures and HIPAA guidelines. Participates in weekly point of care chart closures and huddle discussions. Participates in Quality Improvement Program activities, CQI efforts, and applicable meetings and understands related goals and targets and what can personally do to contribute to positive movement. Effectively and efficiently utilizes EHR to assist organizational CQI efforts, clinical data validation, coordination with external providers, and completeness and accuracy of patient record. SC state law requires the CMA administer controlled or intravenous meds, contrast or chemo agents, neurotoxins, modulatory, or tissue fillers injections, not use laser or instruments for tissue destruction, place sutures, perform radiographs, or analyze/interpret/diagnose symptoms or tests, or triaging. If approved tasks are delegated by Providers, the CMA ensures they are in close physical proximity to provider & within the training and competency of the CMA so long as it won't pose a significant risk to patient if improperly performed. Careteam Plus staff work together as a team and help each other and patients/clients by performing other duties as assigned. Education (Per SC state Law) Meet one of the following criteria as long as the program included medical terminology, pharmacology, medical lab techniques and a clinical experience "rotation" with a healthcare provider:o Completed a medical assisting education program accredited by the Commission on Accreditation of Allied Health Education Programs or its successor or Completed a medical assisting education program accredited by the Accrediting Bureau of Health Education Schools or its successor, or Completed a medical assisting education program recognized by the United States Department of Education or Completed a medical assisting education program through a Career and Technical Education Health Sciences Program approved by the South Carolina Department of Education; or Completed a medical assisting program provided by a branch of the United States military; or Completed a medical assisting program with the United States Department of Labor-approved Registered Apprenticeship program; or Completed a medical assisting training program that is delivered, in whole or in part, by a health care employer that aligns to a nationally accredited certification exam Experience A minimum of one year prior experience with Pediatric, ID or Primary Care CMA direct patient care work required. Experience with Athena EMR preferred. Certification, License, Registration Certified Medical Assistant or Registered Medical Assistant required issue by an entity that is approved by the SC Board of Medical Examiners and the Board of Nursing; is accredited by the National Commission for Certifying Agencies or other accreditation body recognized by the Board of Medical Examiners and the Board of Nursing. National HealthCareer Association American Association of Medical Assistants National Center for Competency Testing National Certification Medical Association American Medical Technologists MedCA American Allied Health (RMA certification) Current BLS certification required.

A solution-oriented mindset required. A strong sense of urgency and focus in accomplishing tasks required. Bilingual fluency in Spanish and English preferred.
