Credentialing:

Core Responsibilities:

Review Medical Documentation:

Read physician notes, operative reports, and patient charts for verification and coding accuracy.

Assign Standardized Codes:

Apply ICD-10-CM, CPT, and HCPCS Level II code sets accurately.

Support Medical Billing:

Process and verify claims so healthcare providers receive proper reimbursement from insurance payers.

Ensure Regulatory Compliance:

Follow healthcare laws, payer guidelines, and privacy standards like HIPAA to prevent fraud and abuse. Participate in internal chart audits.

Resolve Claim Denials:

Investigate rejected claims, correct mismatched codes, and communicate with providers or billing teams to fix errors.

Provider Communication:

Query physicians and healthcare staff to clarify missing, incomplete, or ambiguous documentation.

Coding Education:

Keep up-to-date with annual code set updates and regulatory changes. #NHR2

Pay:
