Job Description

Hospice Medical Coder - PRN/ As Needed This position is an as-needed role that will serve as a backup to our full-time Medical Coder. Responsibilities may include:
Review admission, recertification, and discharge documentation for coding accuracy, completeness, and compliance. Assign
ICD-10-CM
diagnosis codes in accordance with CMS, LCDs, Conditions of Participation (CoPs), and ACHC standards. Sequence diagnoses appropriately to support hospice eligibility and coverage determinations. Collaborate with physicians, clinicians, and interdisciplinary team members to clarify documentation and resolve coding questions. Ensure coding documentation supports accurate claims submission, Medicare billing requirements, and reimbursement. Submit coding documentation within established timeframes to prevent billing delays. Work closely with the Hospice Operations Coordinator and Accounting team to ensure coding-related documentation is processed timely. Maintain coding records, documentation, and audit trails to support compliance and accreditation reviews. Participate in Quality Assessment and Performance Improvement (QAPI) initiatives. Provide education and guidance to clinical staff regarding coding standards and documentation requirements. Maintain patient confidentiality and ensure compliance with HIPAA and all applicable federal and state regulations.
Requirements for this position include:
Current
ICD-10-CM
coding certification required. Experience with hospice coding and home health admissions preferred. Working knowledge of Medicare hospice billing practices and coding guidelines. Proficiency with Microsoft Excel, Word, and other computer applications.
