QUALIFICATIONS
Although, Individual are encouraged to apply, the Ideal candidate with the following experience will only be considered. Must be US Citizen or Authorized to Work in the United States No Sponsorship Available Requirements for Medical Director•Geriatric  Care Completion of a physician residency program accredited by the Accreditation Council for Graduate Medical Education (ACGME). Master's degree or doctoral degree, as applicable to the physician's professional credentials. Board certification in Internal Medicine, Family Medicine, or Emergency Medicine. Geriatric medicine experience, training, or certification is strongly preferred. Active, unrestricted physician license in the applicable state. Current state Controlled Dangerous Substances (CDS) certification, as applicable. Current DEA registration Eligible for or currently enrolled as a provider with Medicare and Medi-Cal Demonstrated knowledge of chronic disease management and geriatric medicine preferred. Ability to work collaboratively within an interdisciplinary care model. Willingness and ability to participate in a rotating night and weekend on-call schedule. Ability to provide care in multiple settings, including the PACE center, participant homes, hospitals, and nursing facilities.

Unlike traditional fee-for-service practice, the PACE model allows physicians to spend meaningful time understanding the whole person—not simply treating an isolated diagnosis. As the Medical Director you will develop long-term relationships with participants and work alongside an interdisciplinary team to address medical, functional, behavioral, and social needs. The successful physician will have an opportunity to: Play a significant clinical leadership role within a growing PACE center. Help shape the culture, clinical standards, and effectiveness of the care team. Build lasting relationships with participants and their families. Practice medicine within a holistic, value-based care model. Influence individualized care plans and clinical outcomes. Collaborate closely with physicians, Nurse Practitioners, nurses, social workers, therapists, and other IDT professionals. Provide clinical leadership across community, home, hospital, and skilled nursing environments. Mentor and provide direction to Nurse Practitioners and other members of the interdisciplinary team. Participate in organizational growth and future leadership opportunities. Key Responsibilities Conduct comprehensive in-person medical assessments and provide primary care to assigned participants. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Provide ongoing medical oversight and continuity of care. Coordinate admissions and discharges involving hospitals, nursing homes, acute-care facilities, and long-term placements. Collaborate with community-based physicians, specialists, hospitals, and post-acute providers to ensure effective transitions of care. Participate actively in IDT meetings and communicate significant changes in participant condition, treatment, and care needs. Coordinate and support delivery of appropriate 24-hour medical care. Provide medical oversight for participants residing in nursing facilities, including regular visits based upon clinical need. Provide direction, guidance, and clinical support to Nurse Practitioners and other members of the interdisciplinary team. Promote evidence-based approaches to chronic disease management and geriatric care. Support appropriate utilization of specialists, hospital services, nursing facilities, and other healthcare resources. Maintain a strong focus on quality, safety, participant experience, and clinical outcomes. Participate in a rotating physician on-call schedule.
