3001565066-2 Apply for this job now Job Description Conviva Senior Primary Care seeks an Associate Medical Director to support clinical leadership across our senior-focused primary care centers. In this role, you will provide direct patient care for adults 65+, guide evidence-based practice, and support quality, safety, and value-based initiatives. You will mentor clinicians, collaborate with interdisciplinary teams, and drive preventive and wellness programs. Ideal candidates are board-certified physicians with leadership experience, a passion for geriatric care, and a commitment to compassionate, patient-first, team-oriented practice. Responsibilities Provide clinical leadership and oversight for senior-focused primary care services across assigned Conviva Care Centers Collaborate with Medical Directors and primary care providers to ensure high-quality, evidence-based care for a senior population Support development, implementation, and refinement of clinical protocols, pathways, and standards of care tailored to older adults Monitor clinical quality, patient safety, and healthcare outcomes, using data to drive continuous improvement initiatives Participate in and help lead quality improvement projects, including value-based and population health programs Partner with operations, nursing, and allied health teams to optimize clinic workflows and patient access Review performance metrics, including hospitalizations, readmissions, preventive care, and chronic disease management outcomes Provide mentorship, coaching, and feedback to clinicians and clinical teams to support professional growth and consistent standards of care Engage directly in patient care as appropriate to maintain clinical expertise and model best practices Ensure compliance with regulatory requirements, payer expectations, and internal policies, particularly for Medicare and senior populations Collaborate with leadership to design and implement patient-centered initiatives that enhance experience, engagement, and retention Represent Conviva in clinical and community forums, strengthening ties with local partners and senior-serving organizations Contribute to recruitment, onboarding, and evaluation of clinical staff, supporting a culture of teamwork, respect, and accountability Help align clinical strategies with organizational goals around sustainable healthcare, cost-effective care delivery, and improved outcomes Communicate effectively across disciplines, promoting a culture of compassion, collaboration, and continuous learning Required Skills Clinical leadership Geriatric medicine Primary care management Care model development Quality improvement (QI) Clinical protocol development Population health management Value-based care Data analysis and outcomes monitoring Electronic Health Records (EHR) proficiency Regulatory and compliance knowledge (CMS, Medicare) Team management and coaching Interdisciplinary care coordination Patient safety and risk management Utilization management Performance metrics and reporting Clinical policy writing Stakeholder communication and presentation Change management Conflict resolution Strategic planning in healthcare
