Job Summary The Assistant Medical Director - Care at Home directly reports to the Regional Medical Director and supports the clinical oversight, quality, safety, and performance of the Care at Home program. The role provides day-to-day clinical leadership for physicians and advanced practice providers, including chart audits, provider training, shadowing, coaching, peer review, and performance improvement. The Assistant Medical Director is accountable for supporting achievement of established clinical, quality, documentation, utilization, and operational metrics; reviews patient eligibility, discharge readiness, and care plans; maintains a small panel of Care at Home patients; and travels as needed for patient care, provider oversight, training, and program operations.   • Provides leadership for activities related to the delivery of Care at Home medical care and clinical services, including quality assurance, utilization review, cost management, patient safety, and clinical outcomes.
  • Collaborates in the development and periodic review of written clinical policies, guidelines, protocols, workflows, admission criteria, and care standards for the Care at Home program and responsibilities of clinicians.
  • Monitors and is accountable for achievement of established Care at Home clinical, quality, documentation, risk coding, HEDIS, utilization, patient experience, and operational performance metrics; analyzes results, identifies gaps, and implements improvement plans.
  • Conducts routine and targeted medical record audits to evaluate clinical appropriateness, documentation quality, coding accuracy, medication reconciliation, care planning, evidence-based practice, and compliance; provides feedback and re-audits as indicated.
  • Reviews patient eligibility for Care at Home services, continued program eligibility, discharge readiness, and care plans to ensure appropriate enrollment, safe transitions, and alignment with clinical needs and program criteria.
  • Leads provider onboarding, orientation, continuing education, in-service training, competency validation, and retraining based on audit findings, quality trends, and identified clinical needs.
  • Shadows physicians and advanced practice providers during in-home, virtual, or other patient encounters as appropriate to assess clinical performance, workflow adherence, documentation, patient communication, and coaching needs.
  • Provides ongoing coaching, mentoring, clinical guidance, and performance feedback to Care at Home providers, including support for performance improvement plans when needed.
  • Leads or participates in clinical peer review and review of complex or high-risk cases, adverse events, readmissions, emergency department utilization, patient complaints, and other quality or safety concerns; develops improvement actions and escalates significant concerns to the Regional Medical Director.
  • Monitors the quality and appropriateness of medical services provided to Care at Home patients and supports corrective action when gaps are identified.
  • Supports interdisciplinary management of medically complex, chronically ill, homebound, and high-risk patients, including medication management, goals of care, transitions of care, and coordination with nursing, case management, social work, pharmacy, operations, and other teams.
  • Partners with interdisciplinary and operational teams to close care gaps, improve care coordination, reduce avoidable utilization, and support safe care in the home.
  • Provides professional oversight for advanced practice nurses (ARNP/APRN) and/or physician assistants (PAs), as applicable, including collaboration or supervision requirements consistent with state law and organizational policy.
  • Participates in the recruitment, interviewing, selection, onboarding, and retention of Care at Home medical providers and other clinical staff as requested.
  • Assesses provider performance, directs clinical work as delegated, and provides input into performance appraisals and development plans.
  • Addresses urgent clinical or operational escalations and patient safety concerns, communicating significant issues to the Regional Medical Director and appropriate leadership.
  • Reviews patient satisfaction trends and clinical complaints and supports corrective and preventive actions when indicated.
  • Supports regulatory compliance, infection prevention, emergency preparedness, and applicable organizational and accreditation requirements within the Care at Home environment.
  • Provides clinical input into Care at Home workflows, technology, care delivery models, clinical tools, policies, procedures, and practice management functions.
  • Participates in medical governance, quality, medical records, infection prevention, adverse event, utilization, and other committees or workgroups as assigned.
  • Participates in and may lead clinical development and quality improvement initiatives for the Care at Home program.
  • Partners with clinical and operational leadership to drive program growth and expansion, identifying opportunities to broaden services, enhance care delivery models, expand patient access, and support the successful implementation and scaling of new initiatives.
  • Maintains a small panel of Care at Home patients and delivers direct patient care within the scope of practice, including longitudinal management and coverage of complex or high-risk patients as needed.
  • Travels periodically to patient homes, clinical sites, market locations, provider locations, and corporate meetings for clinical oversight, provider shadowing, training, audits, or other business needs.
  • Performs other duties as assigned by the Regional Medical Director or executive team.
Supervisory Responsibilities This position provides delegated clinical oversight, coaching, auditing, training, and performance support for Care at Home physicians and advanced practice providers. The Assistant Medical Director may direct clinical work and contribute to provider performance management in collaboration with the Medical Director and operational leadership. Required Education
  • MD or DO degree from an accredited school of medicine. Required Experience
  • At least 2 years of experience as a primary care physician. Prior physician leadership, medical director, assistant medical director, lead physician, home-based primary care, or value-based care experience is preferred.
  • At least 2 years of experience working with a multicultural patient population and delivering patient-centric programs that are high quality, clinically appropriate, and cost effective. Experience caring for medically complex, chronically ill, geriatric, or homebound populations is preferred. Required Licenses and Certifications
  • Unrestricted license to practice medicine in the selected state.
  • Board certified in one of the primary care specialties. Required Knowledge, Skills, and Abilities
  • Any combination of education, training, and experience that demonstrates the ability to perform the duties and responsibilities described, including clinical leadership, provider coaching, chart auditing, quality improvement, performance management, interdisciplinary care coordination, and data-driven achievement of clinical metrics. Preferred Qualifications
  • Experience in home-based primary care, Care at Home, population health, value-based care, utilization management, quality improvement, HEDIS/risk adjustment, or management of medically complex patients is preferred.

Travel Required:
Flexible to travel per business needs, including periodic travel to patient homes, clinical sites, market locations, provider locations, and corporate meetings for patient care, clinical oversight, provider shadowing, training, and audits.   • Prolonged periods working at a computer and participating in meetings.
  • Ability to travel periodically to patient homes, clinical sites, market locations, provider locations, and corporate meetings as required.
  • May spend time in patient homes and other clinical or operational settings; exposure to healthcare and home environments, temperature variation, patient interaction, pets, stairs, and other variable conditions is possible.
  • Comfort managing in a fast-paced, multi-site and home-based healthcare environment while balancing clinical leadership responsibilities with a small patient panel.

Environmental Conditions Inside and In-Home:
The employee is subject to environmental conditions in both clinical/office settings and patients' private homes, which are not controlled by the company and can vary widely in temperature, cleanliness, layout, lighting, and the presence of pets. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level. The employee may also be exposed to outdoor weather conditions while traveling between locations.
