Director of Medical Records Ayers Health and Rehabilitation Center 606 NE 7th Street Trenton, FL 32693 Full-Time | Monday-Friday Previous Skilled Nursing Facility Experience Required Protect the Record. Support Compliance. Strengthen Resident Care. Ayers Health and Rehabilitation Center is seeking an experienced and highly organized Director of Medical Records to oversee the accuracy, security, organization, and regulatory compliance of our residents' medical records. This position is ideal for a detail-oriented professional who understands skilled nursing documentation requirements, confidentiality standards, record-retention practices, and the importance of maintaining complete and survey-ready clinical records. What We Offer Competitive compensation based on experience Monday-Friday schedule Medical, dental, and vision insurance 401(k) with employer contributions Paid time off Company-paid life insurance DailyPay—access your earned wages when you need them Supportive leadership and collaborative work environment Opportunities for professional growth Key Responsibilities Maintain complete, accurate, organized, and confidential resident medical records Review records for missing, incomplete, or inconsistent documentation Track and obtain outstanding physician signatures, orders, reports, and other required documentation Ensure records comply with HIPAA, federal and state regulations, and facility policies Process requests for medical records and release information according to legal and regulatory requirements Organize and maintain active, discharged, and archived resident records Complete audits and assist with ongoing survey-readiness efforts Coordinate with nursing, therapy, social services, business office, admissions, and other departments Support admission, discharge, transfer, and record-closing processes Maintain physician credentialing and other required documentation as assigned Protect resident confidentiality and control access to protected health information Assist leadership with reports, audits, investigations, and regulatory requests Monitor record-retention requirements and ensure proper storage and destruction procedures Perform additional administrative and compliance-related duties as needed Qualifications Previous medical records experience in a skilled nursing or long-term care facility required Strong knowledge of HIPAA and resident confidentiality requirements Familiarity with skilled nursing documentation and regulatory standards Experience using electronic health record systems; PointClickCare experience is a plus Strong attention to detail and ability to identify incomplete documentation Excellent organizational, communication, and time-management skills Ability to manage multiple priorities and meet established deadlines Professional judgment when handling confidential and sensitive information Health Information Management certification is a plus but not required The Professional We Are Looking For The successful candidate will be dependable, thorough, and proactive.
