Job Description
ABOUT US
Founded in 2017, Upperline Health is the nation's largest multispecialty physician group dedicated to transforming value-based care (VBC). We are redefining how care is delivered by integrating specialty care and primary care into a patient-centered and fully coordinated VBC model- enabling earlier intervention, improving outcomes, and enhancing quality of life for our patients. By breaking down traditional silos between specialists, primary care providers, payors, and patients, Upperline delivers a more connected and holistic care experience through Upperline Plus, our VBC program . Our providers coordinate patients' care and collaborate across a multidisciplinary team
- including physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers
- to address both immediate and long-term patient needs.
ABOUT THE MEDICAL DIRECTOR ROLE
As we continue to scale Upperline Plus in our regions, Upperline Health is seeking a Medical Director for the Atlanta market to advance clinical excellence, strengthen provider engagement and lead clinical value-based care across the market (northeast and east Atlanta metro) . This role combines patient care, clinical leadership, and VBC program management and partners closely with the central interdisciplinary team, population health and operations teams. With a patient-centered approach the Medical Director will see a small panel of their own patients, provide clinical oversight and hands-on coaching to Upperline Plus' nurse practitioners, identify market opportunities, improve clinical processes, and lead VBC and population health initiatives- all with a focus on optimizing the care model to improve patients' outcomes and quality of life.
