Lead the Future of Integrated Primary Care
Valley Oaks Health is seeking a Medical Director of Primary Care to provide physician leadership for an established and expanding primary care program across our regional health system. You will join a strong clinical and operational team already in place, with the opportunity to bring your expertise to enhance clinical quality, strengthen provider practice, advance integrated care, and help guide the continued growth of primary care across our 10-county Indiana service area. This is a key leadership role responsible for shaping the future of primary care delivery while supporting our mission to serve diverse and underserved communities.

Your leadership footprint:
2 established primary care clinics (Lafayette & Rensselaer)
4 additional clinic locations planned for 2027
Continued expansion of primary care throughout our 10-county service area
Clinical leadership and supervision of up to 5 Family Nurse Practitioners
40-60% direct patient care, with dedicated time for medical leadership
Full-time work preferred, with possibility of part-time employment
Potential for hybrid/remote work on a limited basis

What You'll Lead
Clinical Leadership & Quality Provide physician leadership for evidence-based clinical standards and best practices across primary care
Drive performance on UDS, HEDIS, value-based care, patient safety, and quality outcomes
Lead clinical quality improvement and peer review
Ensure clinical compliance with HRSA, Joint Commission, and applicable regulatory requirements
Integrated Care Advancement
Co-lead integration of primary care, behavioral health, and substance use services
Support collaborative care models and routine screening initiatives
Promote coordinated care, warm handoffs, and team-based workflows
Help develop innovative care models that improve patient outcomes
Provider Leadership
Lead, mentor, and support up to 5 Family Nurse Practitioners
Establish expectations for quality, access, productivity, documentation, and patient experience
Promote clinical consistency, professional growth, and strong team-based practice across locations
Operational & Access Improvement
Improve patient access through scheduling optimization and panel management
Collaborate with operational leaders to enhance clinic workflow
Monitor key metrics such as cycle time and appointment availability
Reduce no-show rates and improve visit utilization
Strategic Growth & Community Engagement
Support expansion of clinical programs and services
Identify community needs and develop innovative service offerings
Participate in strategic planning and organizational leadership
Represent Valley Oaks Health in community partnerships
Direct Patient Care
Provide direct clinical services consistent with specialty training
Estimated direct care expectation: 40-60%

You will have a capable team around you, meaningful influence over clinical direction, and the opportunity to help expand access to high-quality integrated care across multiple Indiana communities.
MD or DO with a current license to practice medicine in the State of Indiana.
3-5 years of Physician leadership experience preferred.
Meets pre-hire competency and reference requirements.
Holds a valid Driver's License, a good driving record, and insurance.
