Job Description
Our Company Abode Care Partners Overview In this full time, clinical physician lead role, under supervision of the CMO, you will have several clinical responsibilities, along with some specified administrative responsibilities. While providing day-to-day clinical care to residents residing in facilities as well as home bound patients, you will practice and role-model ethical and evidence-based decision-making to promote the vision and mission of the company to other practitioners and staff. Responsibilities Provides high-quality complex and geriatric care to residents and patients across settings in partnership with non-physician practitioner and other interprofessional team members to an identified panel of patients across various assisted living and skilled nursing facilities Keeps high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families Provides collaborating physician duties for on average of 5 but up to 15 nurse practitioners and physician assistants (based on state regulatory parameters) to support them in their clinical roles Based on state and regional regulatory frameworks, audits up to 10% of practitioner medical orders and/ or documentation for each provider monthly If needed, assumes medical directorships for skilled and/or other senior living facilities Helps identify new growth opportunities with skilled facilities and/or other senior living facilities in the market/region Participates in company frontline /backup call processes for timely response to acute changes of condition to prevent avoidable hospital transfers, or to meet other clinical needs Keeps a high focus on quality outcomes including but not limited to care path compliance and productivity, prevention of avoidable transfers and decrease in polypharmacy and costs of care Attends required trainings, and meetings as mandated by the CMO or other leaders to assure best care delivery by the company
