This position combines hands-on patient care with clinical leadership responsibilities in skilled nursing and rehabilitation settings. It is an excellent opportunity for a physician who enjoys caring for medically complex patients and wants greater independence, schedule flexibility, and the opportunity to take on a meaningful leadership role. Physicians in this setting have the ability to build lasting relationships with patients, families, nursing staff, advanced practice providers, and facility leadership while practicing in a more predictable environment than traditional hospital-based medicine. The physician will provide ongoing medical care to patients in both short-term rehabilitation and long-term care settings. Responsibilities include performing patient assessments, managing acute and chronic medical conditions, developing and updating treatment plans, ordering and interpreting diagnostic testing, prescribing and adjusting medications, coordinating care with nursing and interdisciplinary teams, communicating with patients and families, and completing clear and timely documentation. This position includes medical director responsibilities at multiple facilities in addition to attending physician duties. The physician will work collaboratively with facility leadership and clinical teams to support high-quality patient care, regulatory compliance, clinical standards, and overall quality improvement initiatives. Medical director responsibilities are compensated in addition to clinical earnings. The practice provides strong clinical and operational infrastructure, structured onboarding and training, administrative support, and resources designed specifically for physicians practicing in post-acute care. Physicians also have opportunities for continued leadership and professional growth. Compensation Estimated annual compensation is approximately $300,000 to $340,000, depending on productivity and the scope of medical director responsibilities. The compensation model includes productivity-based earning potential, allowing physicians to increase their total earnings based on patient volume. Medical directorship responsibilities include additional compensation. Qualifications Active New York medical license, or eligibility to obtain one No unresolved disciplinary action affecting licensure Non-board-certified physicians are welcome to apply. Location This opportunity is located in the Syracuse, New York area and serves multiple post-acute care facilities within the market.
